Signet Health has an immediate opening for a Social Worker/Case Manager/Discharge Planner at our program with Chambers Memorial Hospital, Danville, Arkansas.
A full and market-competitive compensation and benefits package is offered with this position.
Schedule is Monday - Friday, no weekends, or holidays.
Responsibilities for this position include:
Knowledgeable with the appropriate policies and procedures to coordinate with the admission staff and clinical staff to facilitate the meeting of patient's treatment needs.
Responsible for management of the discharge plan for the patients stay throughout the program.
Contacts referral sources and family members to gather clinical information for the multidisciplinary team and reports to the Case Manager/Discharge Planner.
Interacts with members of the medical/clinical team to provide a flow of communication.
Accurately documents in the medical record the findings and data that supports level and intensity of service rendered.
Assists in facilitating the treatment team process as a member of the multidisciplinary team.
Communicates with patients, families, and referral sources to ensure the positive treatment outcomes.
Maintains performance improvement activities within the department and participates in TQM activities.
Adheres to the highest ethical standards regarding patient care and rights and facilitates communication to outside agencies to ensure that these patient rights, care, and needs are met.
Responsible for facilitating Coping Skills group three times a week.

Qualifications:
Qualifications Needed:
MSW desired; will consider BSW with related experience.
Salary:
$31 per hour.
Estimated Salary: $20 to $28 per hour based on qualifications.
